Our 3-step program

Intake Assessment Process

1. Create an Account: The Initial Intake assessment process requires you to first Create a secure TrueForm GPL-1 Member Login (link).
2. Access Forms: In your account you will see a link for the medical intake forms. Clicking on the “TrueForm GLP-1 Intake Forms” link to access the four (4) required forms.
3. Complete the four (4) intake forms
- a) Intake Form
- b) Weight Loss Consult Form
- c) Medication Consent form
- d) HIPAA Privacy Form
4. Submit each form using the submit button, which sends them securely and directly to your provider.
5. Review/Approve: Your intake forms will be thoroughly reviewed. The clinician will notify you to schedule a telehealth appointment once you are approved for weight loss and GLP-1 medication.

Telehealth Visits

1. Telehealth Appointment: Your provider will send you a secure appointment invitation link from Doxy.me, which is our telehealth platform for visits.
- Note: You don’t need a doxy.me account to join your visit – simply click your provider’s link to start your session
2. Medical Intake and History Review: Your first visit will last 60 minutes and will focus on the following objectives:
- review your medical information,
- determine and set optimal weight loss goals for you.
- develop a successful weight loss plan that includes GLP-1 prescriptions.
3. Schedule a follow-up appointment minimally monthly if you are on a GLP-1, or more often as needed and upon your request for support.

Support Visits

1. Follow-up support visits: Your provider will send you a secure appointment invitation link from Doxy.me.
2. Follow-up Appointment Forms: Prior to your appointment, you are required to complete and submit the TrueForm GLP-1 Follow-up which may be found in your account.
3. Next Visit: If you are continuing to take a GLP-1 medication, regular monthly telehealth visits are required. Missing follow-up visits will result in a discontinuation of your GLP-1 prescription.
4. Additional Visits: If you would like clinician follow-up and support care more frequently, you may request appointments through the Contact Us page using the drop-down request.
